Confidentiality Agreements

 

RELEASE OF INFORMATION

 

The following are examples of Releases that you would be asked to sign in order to satisfy your testing and reporting requirements:

Court Ordered Testing Release:

 

"This consent for disclosure is to be used for Criminal Justice Referrals whose treatment is a condition of the disposition of any criminal proceedings against the client or of the client’s parole/probation or other release of custody.  Information may be disclosed verbally, in writing, or by electronic means to the appropriate referring Agent."

Private Testing Release:

 

This Release and Consent authorizes and gives full permission to have D’n’A send the specimen or specimens so collected to a laboratory for a screening or confirmation test for the presence of any prohibited substances, and for the laboratory or other testing facility to release any and all documentation relating to such test to DNA. I understand that only duly-authorized employees will have access to information furnished or obtained in connection with the test, and DNA will maintain and protect the Confidentiality of such information to the greatest extent possible and will share such information only as directed by the Client.

 

All Clients and testing results are confidential unless each client has signed a release of confidentiality!
