Become a Client at D'n'A!
In order to become a client at D'n'A, you will need to bring the following:
1. $25.00 Registration Fee (*subject to change depending on case*)
2. Picture ID (i.e. Driver's License, State ID, Passport, School ID, etc.)
3. Prescription Medication that could show up on a drug screen.
4. Court ordered paperwork or D'n'A Referral form from Referring Agent.
If you are a Referring Agent looking to refer one of your clients to our testing center to be put on a random color system, here's how!
1. Click the PDF button to Download our Referral Form!!!
2. Fill out form with frequency, testing requirements, and contact information or D'n'A to send all results to! (i.e. Fax number, email address)
3. Have your client bring the following items:
